Blind Man Cassidy enters the story as a blind beggar met on the roads of New Hanover. Permanently blind for unknown reasons, he is a short, hunched man who supports himself with a cane and carries donations in a small metal tankard. His worn clothes, dry grey hair and coarse beard make him conspicuous around Cumberland Forest, Twin Stack Pass and Annesburg. When Arthur Morgan approaches, Cassidy asks him to help a blind man.
Arthur can give Cassidy one dollar. In return, Cassidy delivers an obscure, metaphorical prediction, usually concerning Arthur himself or events still to come. Arthur does not know what to make of the man's words, treating them as strange or amusing rather than clear advice. Cassidy appears repeatedly during 1899, and Arthur can meet him eighteen times as the gang's story unfolds.
In 1907, the point of view passes to John Marston, who can encounter Cassidy eight more times. Cassidy continues asking for a dollar and answering charity with another cryptic riddle. John is no more certain than Arthur how seriously to take the predictions. The blind man remains on the frontier, appearing in places including Great Plains and Tumbleweed in Gaptooth Ridge.

- โBlind Man Cassidy functions similarly to the Soothsayer, whose riddles cost a dollar more and are usually more cryptic.
- โThere's a known glitch that may happen if the player murders Blind Man Cassidy, which makes bounty hunters appearing to come after them regardless of a bounty on their head for the rest of the game.
- โIf antagonised, he may say to Arthur or John, "I see a bullet in your future, you son of a bitch". For John, this does indeed come true at the end of Redemption, except that he took several bullets before dying. As for Arthur, if he has low Honor and helps John escape the Pinkertons in the mission "Red Dead Redemption", Micah will shoot him in the head.
- โBlind Man Cassidy appears to be inspired and modeled after Tiresias, a blind prophet in the Greek mythology. He made appearances in several Greek stories such as Odyssey and Oedipus Rex. Additionally, blind prophets have served as an archetypal trope in various media and modern art.
