Thomas Downes é um fazendeiro de Downes Ranch, em The Heartlands, onde vive com a esposa, Edith, e o filho, Archie. Ele também arrecada doações para The Charitable Organisation of New Hanover, embora sua família enfrente dificuldades financeiras e sua doença esteja piorando. Durante o capítulo Horseshoe Overlook, seu trabalho o leva a Valentine, onde fica perto dos estábulos pedindo contribuições de caridade.
Downes interfere do lado de fora de Smithfield's Saloon quando Arthur Morgan continua espancando Tommy depois de vencer a briga. Arthur o dispensa. Mais tarde, Downes dá dinheiro aos pobres de Valentine e pode pedir uma doação a Arthur, tossindo enquanto fala. Sem condições de sustentar a família e pagar o que deve, ele já tomou um empréstimo de Leopold Strauss.
Strauss manda Arthur a Downes Ranch para cobrar a dívida. Downes diz não ter dinheiro, e Arthur reage com ameaças e golpes contra o frágil fazendeiro. Thomas tosse sangue durante a agressão, e parte dele atinge a boca de Arthur. Edith e Archie correm para junto de Thomas, enquanto Edith pede mais tempo, mas Arthur vai embora exigindo o pagamento.
Thomas morre algum tempo depois da visita de Arthur. Edith e Archie responsabilizam Arthur pela morte dele. Meses depois, quando Arthur viaja para encontrar Sadie Adler e resgatar John Marston, uma forte crise de tosse o faz desmaiar. Um médico lhe diz que ele tem tuberculose terminal. Mais tarde, Arthur escreve que acredita ter contraído a doença pelo sangue do homem que espancou em Downes Ranch.

- ◆Unlike most of the other loan shark missions, "Money Lending and Other Sins III" is mandatory, making it impossible to prevent Arthur from contracting tuberculosis.
- ◆Additionally, when Arthur confronts Downes during this, the player cannot wear a mask (I.E. the Bandana) and if attempting to do so, Arthur will take it off.
- ◆During the fundraising event in Valentine, the dialogue will vary depending on whether the player has completed "Americans at Rest".
- ◆Thomas cannot be lassoed, hogtied or killed. However, he can become a witness and will try to report the player if the latter has committed any crimes near him. Downes can be also robbed with the use of exploits.
- ◆His grave marker incorrectly spells his name as "Thomas Downs".
- ◆In the second trailer for the game, Edith can be seen mourning at his grave in Valentine. In the files, he has an unused grave near this spot that differs from the final version.
- ◆The player can collect Downes' hat by shooting it off his head before beating him, though they cannot keep it as it will not be saved in the wardrobe.
