A arma fica disponível no início do epílogo. Uma caça ao tesouro leva a Rattlesnake Hollow, no norte de Cholla Springs, a oeste de Twin Rocks. Otis Miller's Revolver é encontrado dentro de uma caverna no local. Ele não é comprado de um armeiro nem recebido de outro personagem.
Otis Miller's Revolver comporta seis cartuchos e permite carregar até 100 cartuchos de Revolver Ammo. Ele pode usar munição de revólver Regular, High Velocity, Split Point, Express e Explosive. Como um revólver, é usado como uma arma curta de seis tiros, e o tipo de munição escolhido determina os cartuchos carregados.

- ◆According to the HUD icon, it is possible the revolver was originally intended to have a short barrel.
- ◆It is possible to collect the revolver before the Epilogue, during the mission "Angelo Bronte, A Man of Honor". During the escape from the graveyard, the player must alert and evade the law and then they can head to New Austin since the invisible sniper will be disabled. It is only possible to do this glitch on version 1.0.0, though other methods of acquiring it have since been discovered.
