Gertrude Braithwaite ist die Tochter von Catherine Braithwaite und ein Mitglied der Familie Braithwaite in Scarlett Meadows, Lemoyne. Sie ist entstellt und psychisch krank. Aus Angst vor dem Schaden, den das Wissen um ihren Zustand dem Familiennamen zufügen könnte, behalten ihre Verwandten sie auf dem Anwesen der Braithwaites, statt sie in eine Heilanstalt einzuweisen. Sie sperren sie in einem Plumpsklo nahe Braithwaite Manor ein, wo Isolation und Vernachlässigung sie zunehmend instabil werden lassen.
Während Arthur Morgan versucht, Penelope Braithwaite von ihrer Familie fortzubringen, kann er Penelope zu Gertrudes Plumpsklo bringen. Penelope erkennt die Frau darin als ihre Cousine und erklärt, weshalb die Braithwaites sie dort versteckten. Sie sagt, dass sie nach ihrer Ankunft in Boston versuchen wird, Hilfe für Gertrude zu organisieren. Arthur bezweifelt, dass sie etwas tun kann. Gertrude bemerkt Penelope draußen und nennt sie aus dem Inneren des Plumpsklos eine "bitch".
Im Epilog steht das Plumpsklo noch immer neben Braithwaite Manor, doch Gertrude wurde nicht befreit. Die Konstruktion ist um ihre Gefangenschaft herum verfallen. Drinnen zeigen ihre skelettierten Überreste, dass sie dort stirbt, offenbar an Hunger.

This is supported by her various sexual quotes and her overall aggression, hinting that she may have suffered sexual abuse from male members of her family.
- ◆If the player attempts to inspect the outhouse, or gets too close to the crescent moon-shaped hole in the door, Gertrude will attempt to grab the player through the hole to no avail.
- ◆If Arthur shoots a gun or throws a molotov or dynamite at the outhouse, he will lose honor, but Gertrude will be nevertheless unaffected. But shooting a gun toward Gertrude's direction can also shut her up.
