Gertrude Braithwaite is Catherine Braithwaite's daughter and a member of the Braithwaite family in Scarlett Meadows, Lemoyne. She is disfigured and mentally unwell. Fearing the damage that knowledge of her condition could do to the family name, her relatives keep her on the Braithwaite estate rather than place her in a sanatorium. They confine her in an outhouse near Braithwaite Manor, where isolation and neglect leave her increasingly unstable.
During Arthur Morgan's effort to get Penelope Braithwaite away from her family, he can bring Penelope to Gertrude's outhouse. Penelope identifies the woman inside as her cousin and explains why the Braithwaites hid her there. She says that, once she reaches Boston, she will try to arrange help for Gertrude. Arthur doubts that she can do anything. Gertrude notices Penelope outside and calls her a "bitch" from within the outhouse.
By the epilogue, the outhouse remains beside Braithwaite Manor, but Gertrude has not been freed. Its structure has decayed around her confinement. Inside, her skeletal remains show that she dies there, apparently from starvation.

- โGertrude's insanity and physical malformation could be the result of an incestuous lineage, as the Braithwaites have been known for condoning inbreeding. This is supported by her various sexual quotes and her overall aggression, hinting that she may have suffered sexual abuse from male members of her family.
- โIf the player attempts to inspect the outhouse, or gets too close to the crescent moon-shaped hole in the door, Gertrude will attempt to grab the player through the hole to no avail.
- โIf Arthur shoots a gun or throws a molotov or dynamite at the outhouse, he will lose honor, but Gertrude will be nevertheless unaffected. But shooting a gun toward Gertrude's direction can also shut her up.
