Gertrude Braithwaite es hija de Catherine Braithwaite y miembro de la familia Braithwaite en Scarlett Meadows, Lemoyne. Tiene deformidades y padece problemas mentales. Sus parientes temen que se conozca su estado y que ello perjudique el apellido familiar, así que la conservan en la propiedad Braithwaite en lugar de enviarla a un sanatorio. La encierran en una letrina cerca de Braithwaite Manor, donde el aislamiento y el abandono agravan su inestabilidad.
Cuando Arthur Morgan ayuda a Penelope Braithwaite a escapar de su familia, puede llevarla hasta la letrina de Gertrude. Penelope identifica a la mujer encerrada como su prima y explica por qué los Braithwaite la ocultaron allí. Dice que intentará encontrar ayuda para Gertrude cuando llegue a Boston. Arthur duda de que pueda lograrlo. Gertrude advierte la presencia de Penelope afuera y la llama "bitch" desde el interior de la letrina.
Para el epílogo, la letrina sigue junto a Braithwaite Manor, pero Gertrude no ha sido liberada. La estructura se pudre alrededor del lugar donde permanece cautiva. En el interior, sus restos óseos revelan que muere allí, al parecer de hambre.

This is supported by her various sexual quotes and her overall aggression, hinting that she may have suffered sexual abuse from male members of her family.
- ◆If the player attempts to inspect the outhouse, or gets too close to the crescent moon-shaped hole in the door, Gertrude will attempt to grab the player through the hole to no avail.
- ◆If Arthur shoots a gun or throws a molotov or dynamite at the outhouse, he will lose honor, but Gertrude will be nevertheless unaffected. But shooting a gun toward Gertrude's direction can also shut her up.
