Gertrude Braithwaite é filha de Catherine Braithwaite e integrante da família Braithwaite, em Scarlett Meadows, Lemoyne. Ela tem deformidades e sofre de problemas mentais. Temendo que a notícia de sua condição manche o nome da família, os parentes a mantêm na propriedade Braithwaite em vez de enviá-la a um sanatório. Gertrude é trancada num banheiro externo perto de Braithwaite Manor, onde o isolamento e o abandono a deixam cada vez mais instável.
Enquanto Arthur Morgan ajuda Penelope Braithwaite a fugir de sua família, ele pode levá-la até o banheiro externo de Gertrude. Penelope reconhece a mulher presa ali como sua prima e conta por que os Braithwaite a esconderam naquele lugar. Ela afirma que tentará conseguir ajuda para Gertrude quando chegar a Boston. Arthur não acredita que ela possa fazer algo. Gertrude percebe Penelope do lado de fora e a chama de "bitch" de dentro do banheiro.
No epílogo, o banheiro externo continua ao lado de Braithwaite Manor, mas Gertrude nunca foi libertada. A construção se deteriora ao redor do local onde ela foi mantida. Lá dentro, seus restos mortais mostram que ela morre presa ali, aparentemente de fome.

- ◆If the player attempts to inspect the outhouse, or gets too close to the crescent moon-shaped hole in the door, Gertrude will attempt to grab the player through the hole to no avail.
- ◆If Arthur shoots a gun or throws a molotov or dynamite at the outhouse, he will lose honor, but Gertrude will be nevertheless unaffected. But shooting a gun toward Gertrude's direction can also shut her up.
