Martin Madrazo is a Mexican businessman who presents himself as legitimate while running the Madrazo Cartel in Los Santos. Witnesses against his organisation disappear before they can testify, and his temper makes crossing him dangerous. He lives at La Fuente Blanca with his wife, Patricia, and their son Miguel, while conducting an affair with Natalia Zverovna. Alfredo Huereca portrays him. Madrazo enters Michael De Santa's life after Michael and Franklin Clinton demolish Natalia's deck while pursuing Amanda's tennis coach.

Madrazo arrives at Michael's Rockford Hills home with armed men. Franklin recognises him, and Madrazo takes Franklin's licence before making him explain his reputation to Michael. He beats Michael with a baseball bat and orders him to repay $2.5 million for the damage. Michael later uses the proceeds from the Vangelico Jewel Store robbery to settle the debt. Satisfied that Michael keeps his word, Madrazo subsequently calls on Michael and Trevor Philips for work from his ranch in Vinewood Hills.

Madrazo sends Michael and Trevor after his cousin Javier, who may testify against him. Javier's plane is shot down, and Trevor recovers sensitive files from the wreckage. When Madrazo refuses Trevor's demand for payment, Trevor cuts off his left ear, takes Patricia and drives away in Madrazo's car. Michael later brings Madrazo an Aztec fertility statue as an offering. Patricia returns home, and Trevor warns Madrazo never to mistreat her again. Madrazo accepts the warning at La Fuente Blanca.
