John Marston wakes in a bed at MacFarlane's Ranch, wounded and bandaged after Bill Williamson shoots him at Fort Mercer. Bonnie MacFarlane tells him that she and her father Drew brought him in and paid the Armadillo doctor to remove the bullet. When she asks why he went to Fort Mercer, John says he wanted to give an old friend a chance. Bonnie offers him a way to work off the doctor's bill: helping her around the ranch.
Bonnie leads John outside and gives him a horse. They ride through MacFarlane's Ranch as she introduces its buildings and the surrounding country, including the barn her father built when she was young. After the tour, Bonnie takes John to the house, where he rests in a chair until evening.
That night, John joins Bonnie on a patrol around the ranch boundary. They find rabbits feeding in the garden, and John deals with the pests. At the corrals, coyotes have come after the chickens, so John and Bonnie kill them. With the animals gone, Bonnie sends John to sleep. He hitches his horse outside his room at MacFarlane's Ranch, ending the patrol.

Trivia
- โThe title name is directly opposite of the mission "Old Friends, New Problems" later in the game.
- โIt is unnecessary to shoot the animals, as Bonnie will do it anyway.
- โJohn's injury cost Bonnie $15 USD to treat. Back in 1911, this was quite a large sum of money. In today's money, that bill would be roughly $400 USD (as of 2022) and $511USD (as of 2026).
- โInterestingly enough, despite this being a relatively large sum of money, John does not actually pay her back, as the $15.00 owed is never taken from John's account at any point during the game. However, it is implied from Bonnie in later missions that John's work around the ranch and the subsequent saving of the livestock from the burning barn & the rescuing of her father was far more than what he originally owed to Bonnie, so the debt was considered absolved.
- โDuring this mission, John will be wearing his cowboy outfit, with the exception of the sleeveless leather overcoat, exposing the bloody bandage around his waist from the bullet wound after he was shot at Fort Mercer. However, during the second part of the mission after he wakes up in Bonnie's house, he is wearing the sleeveless overcoat, therefore hiding the bandage.
- โOccasionally the bandages on John will show blood trying to seep through, seconds later looking as if there never was blood. This happens back and forth, possibly between cutscenes.
- โAt the start of the mission, when walking, John will gasp and hold his wound with his hand, showing that his injury is still very painful.
- โThis is the only time in the whole game that John is barefoot.
- โJohn seems to have a bandage on his right foot. It is unknown if this was from Fort Mercer, from working on Beecher's Hope, or from his time in the Van der Linde gang.
- โIn the original version, Marston's horse will be a Cleveland Bay. However, in the Game of the Year Edition and in the remastered editions, it will be a War Horse instead.
- โIf the player is idling and doesn't get on the horse to go on patrol with Bonnie, the subtitles will incorrectly display one of her lines as "Get on your horse, Mr. Marston. We've got to move this along." instead of "Get on your horse. We've got to move this along."
- โAnother instance of the subtitles being displayed incorrectly is if John doesn't follow Bonnie around the ranch where the text "Try to keep up, will you, Mr. Marston!" is displayed but Bonnie says "Try to keep up, will you!"
- โWhile touring the ranch with Bonnie, she will state that the barn on the property was built by her father when she was little. This directly contrasts the rendition of the ranch in Red Dead Redemption 2. If the player visits the ranch in either Red Dead Online (1898), the main storyline (1899) via glitches or in the Epilogue (1907), the barn does not exist.
